El Centro Care
for our highest risk patients
A program created in collaboration with UNM Project ECHO
to care for our highest risk patients. The patients suffer from poverty, complex and uncontrolled chronic medical illness, mental health disorders, and addiction. This intensive model of care provides 24 hour access to the primary care provider, case management with community health workers, behavioral health care, care coordination and follow up, and home visits to assess living situations serving as barriers to health care and bring care to patients that can’t otherwise travel to the clinic.
